Understanding Gall Bladder Stones

Gallstones (cholelithiasis): cholesterol stones (80% — supersaturation, nucleation, hypomotility), pigment stones (20% — haemolysis, cirrhosis, biliary infection). Risk: 4 F's (female, forty, fertile, fat) + family, rapid weight loss, T2D, insulin resistance, Crohn's, TPN, medications (octreotide, ceftriaxone). Most asymptomatic (silent stones). Symptomatic: biliary colic (RUQ/epigastric pain 30 min–6 hrs, post-prandial, nausea), acute cholecystitis (colic >6 hrs + fever, Murphy's sign, leukocytosis), choledocholithiasis (obstructive jaundice, cholangitis, pancreatitis), gallstone ileus. Diagnosis: ultrasound (stones, wall thickening, pericholecystic fluid, CBD diameter).

Homoeopathy treats the constitutional lithogenic biliary terrain — remedies selected by pain character (colic: severe, RUQ radiating to scapula/shoulder; dull ache), modalities (food, fat, position, heat), associated symptoms (nausea, vomiting, bloating, intolerance to fats), recurrence and miasmatic sycotic/gouty tendency. Treatment improves bile cholesterol solubility (reduces saturation index), enhances gallbladder contractility (CCK response), optimises sphincter of Oddi function and prevents stone growth.
